I believe what's happening here is that the account setup is failing due to illegal characters in the user's name. Probably Cyrillic, given that the image is from a site in Cyrillic.
Assuming that's the case, I'll see if I can make it output a more understandable error message rather than failing in the pictured manner.

I've updated the PDXAuth extension. It should now give the following error message in this case:
"Your forum username cannot be used on the Paradox Wikis due to containing characters not supported by the wiki software."
