I've seen betrothals referred to as being broken in the game, but I haven't actually found a way to do it. How does a betrothal end up being broken, and how much can you as a player do about it?

It's an option in the diplo screen for the betrothed. So if your daughter is betrothed to duke x, but you don't like duke x, go to you daughter's diplo screen, the option should be right above educate child.
Or do you mean the mechanic is too powerful? I guess it could be abused, in theory you could keep betrothing kids to a duke to ensure he never actually marries or has kids.

Option 1: have one of the partners dead.
Option 2: marry one of the partners to someone else manually.

Edit: oh, there is an interface option for it. :)
