Same reason Sweden starts with cores on a bunch of Danish provinces. According to Paradox, everywhere in the present-day boundaries of Sweden + Finland has been rightfully Swedish since time immemorial

PDS once stated that those cores are to make Sweden focus on the rivalry with Denmark. Which I can understand; though why Denmark or Norway don't have cores in Sweden then...

Can't recall the names but there should be different danish, swedish and russian names for this city too. I guess you might know them, since you got the estonian flag in your profile.

I don't know if there used to be an earlier version, but at least in the 1700s and forward the Danish name for Tallinn has been Reval.

Why don't just do "Historic Rival" modifier on their relations. I dont know, but maybe that will ensure rivalry

This and historical rivalry is just a flat opinion malus; it doesn't ensure that Denmark and Sweden gets bogged down in umpteen wars. (Think we had something ridiculous like 60 wars over 1000 years.) That Sweden is overpowered compared to Denmark is something else, but that is far from new.
