Not sure if this is a bug/oversight or working as intended... playing a republic I recruited an Egyptian Lagid with the bloodline trait. I eventually got an event that allowed him to form a new family and displace an older family. The game then generated some characters as part of that new family, including a male child. Unfortunately male children generated like this don’t get the bloodline trait passed on.

I'm guessing that they're adopted characters, but I could be wrong.
No, they are. The event even specifies that "Novus Homo's family will adopt several minor characters", so they're all adopted. Bloodline will be passed on if the trait-holder himself has children through a wife, which is at least possible now that he has a major family.

Not adopted... in addition to the adopted characters, the game generates a male child and female child, both descendants of the character involved with the event (and with lineage displayed on the character sheet).
