There needs to be a way to add turn lanes in some or larger intersections. Many main roads here in Oz have traffic lights and turn lanes that bypass the lights.
Real roundabouts without intersections would be nice.
For that matter I like to modify some intersections like a T intersection - ie have a stop sign only on the side road/street. That way traffic is uninterrupted on the main road.
There also needs some tweaking to motorway on/off ramps. I would like to do nice curved ramps, but it won't curve them the right way.

edit:
Just thought a lot of problems would be solved it you could modify the intersections like said above.
With a T intersection having a stop or giveway sign only on the side road/street without a full blown intersection. This works for roundabouts and motorway ramps too.
Traffic on the main road would not be interrupted.

edit:
While I think of it - a minimap would be good too. :)

An example of a lane bypass is on highway A1A in Florida. The lane on the beach side (heading North) on some of the T intersections have a constant green arrow to allow traffic to continue while the other Northbound lane has a red/green light. (Both southbound lanes and side road also have the red/green lights.)
This would speed up traffic significantly.
