As it stands right now, A pilot is injured and not on duty for X number of days.
I think it would add, as another measure of difficulty, to fine tune what that medical status is. I suppose we are assuming that a pilot must be in perfect health until returning to the cockpit.
What if sometimes you have to have a pilot now? The "Doctor" will recommend that the pilot remain in the med bay until fully healed. But the company commander can override that and put the pilot in action. Of course this means that they are in a weakened state. One measure of difficulty would be that they have less health points. Also it could be a direct affect on their Guts rating. You can't push the heat limits if you're not at 100%
Also, pilots that take too many serious injuries can no longer safely pilot a mech. They are medically retired. Which COULD be something that happens to the Players character. You're not dead, but you're never in a cockpit again. Also, a Company Commander could override the medical retirement but this comes at a cost. It will take twice as long for medically retired pilots to recover from their wounds. Also, they are so beat up they are permanently reduced by two health points. And finally if they are incapacitated in any way, 100% they are dead.
I final suggestion is new scars on pilot portraits. Glitch just got knocked out of action for 90 days? She's going to have a wicked scar on her face from that fight.
The Doctor in this is just needed as a face for how the player interacts with the game on these decisions. The Mechbay, engineering, and nav all have a face. Med bay could get one too.
