Liberate Smithon Mission will not trigger

Please explain your issue is in as much detail as possible.
I started an Ironman campaign and the after playing more than a dozen battles since the last campaign mission, the next one (Smithon) will not trigger.

Anyone who has run into this problem, I was able to get myself put back together by following these helpful instructions (altered for where my campaign broke).
