The crash is in DirectX 11, which points to the video system here. Unfortunately your HD5700 is below the game's minimum requirement of the HD 5870, which. itself can struggle with the game!

All you can really do is turn off Ambient Occlusion in the game settings, and run it at the lowest resolution/graphics quality settings you can stand.

Good luck with that!

Yeah this is what i don't understand with this game, i can run games with my PC that got way higher minimum requirement than this game without crash or freeze and just alot of lag. (Fallout 4, GTA 5) . When i play this game with everything on lowest it run super smooth so thats why i don't understand why i get those freeze.

This is a Unity game, which can mean less than wonderful performance on marginal hardware.

Did the setting I suggested help at all?
