Hi, I see a bunch of people have issues with fully exploring The Abyss, but I have yet to find someone else with the exact same problem as I do.
I'm having trouble accessing this final room in The Abyss in the farthest NE corner:

I've completed the main quest and have chosen the new leader of Bastard's Wound already, yet this room is still unavailable to me. I have opened every door except this one, using torchkeys from the Twin Rivers Oldwall Ruin and the Oldwall Breach Ruins to open those 2 outsider beacons.
I was looking around and apparently I need to have completed History of the Oldwalls Pt 1 given by Lexeme to bring Reef-Talon to the 4 (?) murals around the ruins. I am pretty sure I have visited all 4, definitely 3 at least, with Kills-In-Shadows and/or Lantry in my party and had some thorough dialogue. Lexeme is no longer available to talk to so I don't know if this was a quest I was supposed to have received earlier or only if I spared her, and the murals all no longer have exploratory dialogue when I return to them.
Is there any way for me to progress or have I bugged out my quest order? If there's any way you can help, I'd be very grateful!!
