What speed is it usual to run MP games at ?
Depends on how busy it gets, also if there are alot of people in the game slower helps with lag usually.
Normal is a good speed for most times, during the begining of a game or session, or when there is a new war then usually things slow down for a bit until players get a handle on what is going on.
What is common etiquette for things like pausing the game?
Pause happens for a variety of reasons, basically if someone pauses they should let people know a little about why (i have to kill Germany, something weird happened, Phone, bathroom, aliens invaded earth...).
