Barons, mayors and bishops aren't appointing their courtiers to council positions. Instead, they calculate their state stats by counting themselves again in the "councilor" part of the equation. You can end up with an imbecilic bishop that everyone hates because he refuses all diplomatic advice and a high stewardship mayor who can not only do a good job managing your lands, but also do an awesome job with several cities he runs by himself.
