I mean I don't have statistics for this but in Crusader Kings 2, the code that handles the transfer of a title is hugely complex and is the bane of every programmers existence and is a major culprit for most bugs and even if not immediate cause for the bug has at least a finger or two in it's creation..
If it is 80%? Don't think so, but there is definitely "hot spots" which are common birthplace of issues.
