Are Intel dice rolls working as intended?

No matter what my dice roll is I always earn one insight on an intel operation. I have a difficulty of 5 in this example so I shouldn't be able to make any progress if I am reading these thresholds correctly. Can anyone tell me what I am missing? It's not just giving me unearned insights, but I cant make more insights on good dice rolls either. It feels like they had it set to progress 1 insight per dice roll for testing purposes and never switched it back to release? Does anyone else experience this?

This is my experience as well. Its odd, since its the same mechanic as Archeology and that gets fails and negative results. I've never gotten one in Espionage and neither has anyone else I know.
