Crossings right now are a huge factor in battles yet they are an almost complete mystery. No one has any idea where they are or anything. I seriously hope something is done about them because it's infuriating.

If the graphical movement arrow crosses a graphical river, there will be a crossing penalty.
Alencon -> Paris: No crossing.
Any other prov -> Paris: Crossing.
That is not true in 1.7. We'll see if it is in 1.8.
Not only is it possible to get a river crossing with no graphic crossing the river, it is also possible (albeit very rare) for the arrow to cross a river and not draw a crossing penalty.

It's hard to name the provinces off the top of my head. I have seen it in India (I move from a province with no river into a province with a river on the opposite side of the enemy --> crossing), somewhere near Vijay's capitol but I forget which province specifically. Wolof --> Cayor isn't a crossing. In previous patches I have attacked across a river in Europe fully expecting a penalty and got none in France, assuming that was due to a huge maneuver advantage but not certain. It was noteworthy because I had taken a crossing penalty with the exact same movement previously, but 1.7 changed the rules of terrain choice in general so I'm not sure exactly how it works this patch.

Do you mean that you do get a river crossing there? The movement arrow for Cayor <-> Wolof doesn't cross a river.

Counter example in Scandinavia
Try to move from 19 to the one to upper-right (1755 if I'm not mistaken), arrow crosses river, but you don't get crossing penalty.
Also it's not simply related to one of province positions (army, etc), tested that by playing with positions.txt. At the moment the only sure thing we can say is: if there's a river in start or end province, you may get crossing.
Personally I assume it's somehow related to distance of river from border, but who knows.

Just... How does it work? If province X has the river crossing icon means that ANY army attacking it will suffer the penalty?
I would speculate that it's just an icon with a tooltip naming which provinces will cause crossing penalties
