Anyone knows how to get Eternal Suffering achievement on Steam?

It has smiling Cairn icon, so I assume it means he needs to stay alive somehow. At what route is this possible?

Rebel route. Choose to go recruit the Stonestalkers and help them defeat the Earthshakers.

bumping this thread to inquire whether it is mandatory to do the Rebel path to get this achievement?

It's fairly easy to let Cairn live on any path and kinda silly to lock the achievement behind a specific path if that path is not the only way to do it.

I've just kept him alive in the anarchist path but didn't get the achievement

First you have to be on the Rebel Path, meaning you team up with the vendrian guard.

Second, you will have to go recruit either the Forge Bound or the Bronze Brotherhood (BB) in Lethian Crossing. Your aim is to recruit the Bronze Brotherhood.

Third, the BB will propose to you to recruit the Stone Stalkers in the Stone Sea. Here you go and tries your best to help them without trying to be a prima yourself or confronting the prima, at the end of their quests, you will have to kill the Earth Shakers to get to Cairn. When that is done, go to cairn and click on the hand icon, and you're done.
