On this topic, how does being homesexual hurt? My heir was homsexual, my King dies early, like 42 yo, and so I found a homosexual wife, hoping I would not have 9-10 kids, well I still had 9 kids, and it does not seem to hurt no matter what!!!

Well, it gives -15% fertility, but since there are many traits that can increast fertility -15% isn't really that much. I remember having a homosexual heir as Byzantine Emperor and worrying that he might not produce a son, but he ended up with 7 children. What I should have been worrying about was his 4 mortal sin traits, but that was in my early days of CK2...

There should be some sort of negative relations hit that comes with it but there needs to be a difference betweenmosexuality and "in the closet", with "in the closet the hide or repress there feelings and still have children and try to be streight, while with open its widely known that they are gay and take lovers and never have kids.

I don't think "openly gay" really existed in the timeframe in Europe, at least for public figures. The closest you would come would be widely rumored, like Edward III/Piers Gaveston but they both had wives and kids. And from a reputation standpoint (and I think the traits in general should be viewed as indicators reputation rather than actuality), whether they take lovers or not is irrelevant.

Well I am pretty sure that most royalty could "do" their duty regardless of their sexual preference, so kids should not be that big an issue. Remember some royal couples slept together only now and then just to try for offspring.

I think if your king/queen does have the homosexual trait you should be able to take a lover though. This should lead to the possibility of being outed and the fallout that would occur with automatic excommunication perhaps? Would be an interesting event chain, maybe you have to excute your lover?
