Does anyone actually does that knowing it will happen?

Changing the ethos in many scenarios are just bad, I never do The Old Gods quests neither the horizon signal to the end.

You don't have to change your ethics. If you are already Fanatical Spiritualist, then the Old Gods event doesn't actually change anything. As for the Horizon Signal, when you get The Messenger, just force them to release their funding and don't proceed with that branch of the quest. It used to infuriate me until I realized you could opt out of it.

These events used to be useful if you wanted to switch out of fanatic pacifism after starting with inward perfection/agrarian idyll.
With the changes to these civics that use is gone though.

Yeah. It's fun. My people went from materialists to spiritualists and then, I got the worm event not long after. Which was hilarious and of course, I followed through to the end since it was clearly a sign. My people have deviated far from what they started out as.

The best part was when I agreed to let loose a retro virus that changed my portrait.... to the same thing as my hated enemy (of course, the worm event started in a system I jacked from them, so I don't know if that was intentional). The enemy would continue to call me up and tell me how ugly I looked.

As a matter of fact, the old gods one doesn't have to change anything regardless. You still have two (I think) options at the end that won't touch your ethos.

The Horizon Signal event doesn't change your empire's ethics. It puts all of your pops into a militarist faction if you are not militarist, or puts them all into the pacifist faction if you are militarist.

Old Gods can move you one step towards Spiritualist, but that is one option among four.

If you are Xenophobe and share a planet with pre-sentients, an event can make you Fanatic Xenophobe, but there is a different option that leaves your ethics unchanged.

If you go with Psionic Ascension and have a Chosen One, an event can change your ethics, government type, and civics. This also is optional.

In short if an event has an option to change your ethics it also has at least one other option that does not.
