Asking for an ironman game where I want to know the mechanics before I proceed irreversibly. If a country is marked as co-belligerent in a war that you start and the co-belligerent's allies do not respond to the call, does that break their alliance the same as if they had been the primary belligerent?

It should.
You can always copy the save file, put it somewhere safe, go ahead with the war, and then if it doesn't work as planned put your copy of the save file back in the savegames folder.

FYI, for any one wondering, it did not. Their allies refused the call and remained their allies. I let it slide since it was a live-streamed Ironman, but if anyone else ever is in the same position, it does not work.
