yes, bug confirmed.

Does 'dishonouring' DOTF lower trust? Never realised that. Trust with who? The nations attacked? All same religion nations in that continent?

All nations. After spamming dishonors I had -47 towards alliance due to "trust" with every independent nation I checked. I also have the minimum trust with my march, Castile, which I had kicked off the continent and had been in South America for over half the game. It doesn't affect opinion but it's a modifier applied towards alliances, CTA, and liberty desire. If I wasn't already a runaway it would have been a significant detriment.

Im assuming all same religions as TMIT recalled his vassals becoming disloyal because of it, damn thats harsh

They didn't become disloyal because I was so powerful, but they did receive around +25% liberty desire. I would need to check again, but I'm pretty sure the trust demerit is global. I could be mistaken though.

I think you might be on to something here. Yesterday I had a somewhat weird experience as Muscovy while guaranteeing a nation - I thought if I guarantee Mamluks (couldn't ally them at that time) it would stop Ottomans from expanding into Levant and Egypt. Ottos were allied only to Tunis with no vassals, I had 4 decent-sized vassals and was 1 mil tech ahead, and together we roughly outnumbered them 2:1, only my manpower was quite low and theirs was full. However Ottos DOW'ed Mamluks the moment they got the Levant mission.

I know we're talking Ottomans here, so it's not really clear whether their DOW calculation included my guarantee or not. But it was still a bit of a wtf? moment for me.
The ai probably say your manpower and thought you would decline CTA or be useless... Too bad they miscalculated...

IIRC it even applies to the country who actually declared the war.... not sure about this though.

I had to look this thread up to bring it back up to the front page for one more go. In my current game I claimed Defender of the Faith and I saw a small Catholic nation declare war against a (smaller) Protestant nation completely ignoring the immense Italy that happened to have 41,000 soldiers stationed adjacent to their capital because of another DotF war (and who had already kicked their teeth in during a previous DotF call). Not their best strategic move.

I'll also co-confirm this and add an additional statement that it does seem like the AI just doesn't consider certain alliance scenarios. Norway declared by itself an imperialist war against Kedah in SE Asia, seemingly ignoring the fact that it was a protectorate of France. And that I had a PU over France, calling me in. And my other 3 large PU subjects. Even when he called in his two HRE minor allies, all that did was get them crushed aswell.

To add to what ZangoZett just said, I saw Naples declare war on the Papal States who were allied to Burgundy with all their starting PUs as well as a PU over Castile. Naples had no allies and of course they got absolutely destroyed.
