So I was playing as Hungary trying to form Austria-Hungary and get all the lands needed to get the achievment, then I saw the Requirements list in the in game Achievement tab....

I can't get a screenshot atm, due to steam community being unavailable,

But the list includes:
Jordan, Palestine, Tuscany, Lombardy, Lorraine, Oberschlesien and Niederschlesien, in addition to Emilia Romagna. This is after getting lucky and having annexed the Czechs by event. While Emilia Romagna is actually historically part of the AH-Empire, I don't think Palestine was ever considered part of it.....

Did some of the province codes get changed with the 1.4 update?

Okay, so that gives Jordan and Palestine a reason, any idea's for the Tuscan, Lombard and Lorraine provinces?
Lorraine was apparently once a part of the Austrian Empire (not Austro-Hungarian) and Tuscany, Lombardy, and Emilio Romagna were once part of the Holy Roman Empire. The achievement feels more like restoring the Austro-Israeli-Holy Roman Empire than the Austro-Hungarian Empire, but whatever.

In terms of what the various required provinces are specifically referencing, that would be claims belonging to the House of Habsburg-Lotharingen over lands they or dynastic relations directly ruled over, rather than just a reference to the HRE (which would require far more provinces, actually). Lorraine specifically has to do with the Empress Maria Theresa's husband Franz Stefan, who was Duke of Lorraine (Lotharingen) and was the vehicle for Habsburg retention of the Imperial Crown since whether a woman could be Holy Roman Emperor was disputed. The Habsburgs had claims on Lombardy going back to the Spanish and it was part of the Austrian Empire prior to Italian Unification. A branch of the Habsburg dynasty ruled in Tuscany prior to Unification. Venice was incorporated into the Austrian Empire following the Napoleonic Wars.

For reference, here is the full regnal title of the last Emperor of Austria-Hungary, Karl I, taken from Wikipedia (so possibly wrong in some particulars):
