(Norse English custom nation, no mods though. And my starting leader and heir were both female, which as it turns out means the normally generated heirs are all female as well)

Unbound by strict doctrines our ruler encourages everyone to try new things and reason freely about where to take our great country in the future.
So obviously she is not going to play by the same rules as other monarchs, this is the most reasonable explanation probably.

When you play with a custom namelist which gives you a high chance of reigning queens, strange things will happen ... male consorts with female names are the norm if your ruler is female. My guess is that the game ignores the algebraic signs when creating consorts for queen-regnants (they are also still called 'queens' internally).

Let's hope for 1.23 ... I usually use save-game-editing to give them a more appropiate name.

I'm not using a custom name list, the names are English culture names since my nation has English culture.
I've had one male ruler so far, Reginald became emperor after the previous ruler died without a heir.
