Do I have to completely own and control all the appropriate provinces or can a subject own them and still qualify? Also, do I need to own the islands off the coast like Madiera, St. Helena, and all those islands off Madagascar?

The achievement requires you to own and have a core on the provinces so vassals are a no-go. As for the islands, you need the ones around Madagascar, the two little ones around Benin, plus Saint Helena, Cape Verde, and the Tunisian island Djerba on the North African coast, but not the Canary Islands or the Azores.
