Historically, we know one very important thing:
Military orders were not made so that Christians could win the First Crusade. They were established to protect the winnings after the winning was done by secular crusaders. The current situation in the game is basically a crutch solution with the orders providing the punch and the numbers.
Also:
The First Crusade wasn't really under any sort of control from top lieges, and it wasn't really as simple as dukes from Lorraine/Tolouse/Narbonne/Taranto joining. The leaders were simply leaders, not owners of the troops.
Hence, I'd like to propose something similar to MTW crusades: an adventurer stack with reduced attrition immunity (mortification, perseverance, not to unnatural levels), increased morale, more access to Religious Fervor tactic. The numbers in the stack would depend on how much individual rulers agreed to contribute through events. The leaders would be drawn from the ranks of participating rulers, with AI being reluctant to commit anybody above the ducal rank.
In practice, this could work like leading troops for your liege — you do get battle-related events and may even be asked to pick your command trait when selected as commander for your liege's army. But you don't get to move your liege's troops.
Only after this crusade should the military orders be established and maybe not all five in one go. Templars — as their name suggests — should require control of at least one county or relevant barony in c_Jerusalem, as they were named after the Temple Mount. Hospitallers and Teutonic Knights should be delayed further.
Further crusades could involve monarchs either joining in with retinues or going with demesne levies, or even realm levies. Military order stacks should spawn together with k_Jerusalem army in general, as they should be based in Palestine, not really in Rome or a European castle.
