Is it possible to add and remove counties from duchies? I tried going into the landed titles file to play around with it but I had no luck. If this isn't possible then how could I add or remove counties to create a province (the province being a duchy)?
Adding counties to the de jure duchies?

You can indeed add and remove counties from duchies in landed_titles.txt. What goes wrong if you try it ? It is just a matter of cut and paste in a text-file.
You can't add new counties to the map that way, to create new counties, see this thread.
