So here my story.
I control a female ruler (thus I used the seduce focus to have some of my dukes as lovers
-they must really love my dwarf queen). Now my every adult vassal is member of the autonomy faction (even those who have 100 opinion of me - there really should be another faction for them to join) and while no other vassal has a negative opinion of me (at worst at 0) there is a notable exception the leader of the faction who hates me (-100 opinion who's a pretender and my uncle and the most powerful of my Dukes with 2 duchies).
So on their next meeting they decide they want more (the same happened their last 2 meetings that I accepted) so I say no and they decide to rebel against me (even my 4 duke lovers - all 100 opinions) only a imprisoned duke and 2 under-aged dukes remain in my side.
Other than that They also raise a super army of 70k (when even at my best I had 50k) against my petty 20k army (broken in parts) plus a loan paid mercenary force so I naturally lost the war.
I really think they should decide individually whether they'll revolt or not and not automatically as a whole faction (as was historically accurate).
