About Egypt: Why 'Ptolemy' instead of 'Ptolemaios'?

Because that is how the English speaking world knows him, and the game is in English. It's a bit inconsistent considering that, as you say, some Greek is not translated, but that is how the English language tends to do it. In the game he will be Ptolemy.

As a Swede I am not really used to adapting names to my own language like that so you can be sure he's Ptolemaios in mind. ;)

Trin Trigula is kinda notorious for making these weird decisions all the time. It's like having India with 0 tribe and 1 sect (and not even a historical one) in CK2.

I am sorry to disappoint you but I have had 0 part in who is tribal or not in CK2. The only thing I have done for CK2 India was the map itself, some character research and later some pilgrim events for Jains, Hindus, and Buddhists. :)
Feel free to hold any eu4 decisions related to me against me though, that game I worked on for years (compared to in total over the same period being lent a month or two in total to ck2). ;)

No, its not that hard to grasp. But that doesn't change the fact that English uses:
Philip
Alexander
Ptolemy
Cassandra

It just does. It doesn't make sense, but then nothing much in English makes sense. The "Ph" in Philip and photograph should clearly be an "F" yet still everyone persists with something that even English speakers find strange because: convention. English is pretty much a mess. And if this were a discussion about what is the correct historical names it might matter... but its not that discussion... it is the discussion about what to use in a video game. And in a video game, if you have to choose between "Use the standard language convention" or "use the non-standard language convention", going with non-standard language conventions is a good way to annoy people.

Pretty much this. Even academic works mix and match English names and actual Greek ones much of the time for these specific people. We just follow suit there :)
