For Summerhall, I think it might be a poignant little easter egg to give it the CoA of Ser Duncan the Tall (in houseless hedge knight arms), since that's supposedly where he dies.
In the Reach, Oldtown should have the Hightower CoA, while Goldengrove belongs to House Rowan and should have their CoA. Since the Reach isn't really the name of any specific part of the region I dunno if you'll find a suitable CoA... Maybe give it the CoA of House Gardener, since that's the house that ruled before the Tyrells?
You could do something similar with Lannisport, maybe give it the CoA of a vanished house like Castamere?
Dragonstone could have either the Targaryen arms if you don't plan on bringing them in elsewhere, or maybe Stannis's personal Baratheon arms with the stag in the flaming heart.
King's Landing could maybe have the white Kingsguard shield... Don't know of a particularly appropriate one for the Kingswood, but one cool one that's not often used is the arms of Aegor Rivers in personal arms (he was Bittersteel, the chief ally of Daemon Blackfyre the rebel).
For the Riverlands, yeah I'd use the Mudd arms for Oldstones, and maybe Fisher for Stoney Sept since they're another previous kingly house. Not sure about High Heart. I just realized you don't have Raventree Hall (Blackwood) or Stone Hedge (Bracken), which might be problematic since they're two of the biggest riverlands vassals. Maybe they could replace a couple of those provinces, but if you don't want to do further map changes at this point that's understandable.
For the Bloody Gate, perhaps you could use the arms of the Royces of the Gates of the Moon... a different gate, but still.
I can't really find any info on Sunkenwood or even when it's mentioned in the books. Suppose could use the Lynderlys (they're of Snakewood).
With the North, yeah I'd just use Mountain Clan CoAs where appropriate and such. Fenn looks like it might be good for Moat Cailin. Some random other Northern CoAs should work for the others.
A fully black CoA for the Wall would be cool, especially if you mirrored it with a white one for King's Landing.
Sounds right for Beyond the Wall - even simple patterns would be fine.

On religion - does CK even include Judaism? I'm not sure how it works to mess with the religions. One option would even be just using Christianity (maybe with Orthodox for the North/Iron Isles) and then using traits to represent the others. If you know how to do it better though that's cool.
I wouldn't have a province for the papacy if it can be avoided, since Oldtown is run by House Hightower not the Faith.
