In my game I am a Shia ruler who has conquered most of the Abbasid lands and subjugated the Sunni Caliph as my vassal with only one county.
Still, it seems I can´t destroy the Caliphate or revoke the Caliphs last title, although I am his liege and my religion is hostile to his.
Apparently only forming the Outremer Empire can destroy the Caliphate, but similar possibility should exist for other rival religions, like Shia or Orthodox too.

Honestly, revoking a religious head's last title shouldn't be restricted. I honestly don't understand why it is. You can just grand independence to the religious head in question and then get a claim on their last county and go to war for it and take it away that way.

Pretty sure if you unland the Caliph it destroys the Caliphate title. I seem to remember it working like that I an Outremer run I did on the last patch.
Well, the game does not allow me to unland him once he is my vassal, even when I held a hook on him.
I went around the issue by releasing him and re-conquering via Holy War casus belli as Matihood1 suggested above. Something still felt to be a bit off here: I conquer the Head of Faith, and Ashari emirs and sultans in Egypt, Jerusalem, Persia don't bat an eye. Sure they are less powerful than me, so they might not join the war directly, but they should at least get a quite heavy opinion bonus against me (Though the bisexual Egyptian ruler fancies me since I am a seducer, but that is an other story...)

You should be able to revoke any county/duchy/kingdom title from any direct vassal if you high enough crown authority, even without a claim. I believe the only things that would stop are giving some a title recently, being allied or them having a strong hook on you.

The AI will never join a war unless they are allied with the target. Technically, any ruler of the correct faith can join a regular Holy War, and the game will warn you of that, but, again, only the target's allies will ever join the war. But, generally speaking, the devs don't really seem interested really throwing up strong hurdles for the players. I'm doing an Italy 1066 run and took Rome from the Pope, got excommunicated and had no troubles whatsoever from being excommunicated. If I remember correctly I even managed to get the excommunication lifted from the same Pope who I took Rome from.
