I have some ideas on how to improve the restoration of the Roman Empire.
1. Creating the Western Roman Empire. If you own the Empire of Italia, the Kingdom of Africa, and a second empire title as well as Italia, you should be able to restore the Western Roman Empire. All territory that you hold that was historically part of the WRE would become de jure part of it. (So, Italia would be destroyed and replaced with WRE, Africa would move from Arabia to WRE, and any other Empire or Kingdom rank titles you control that were part of the WRE historically would become part of it as well.)

Titles required to form the WRE:
Empire of Italia (held)
Kingdom of Africa (held or vassalized)
Duchy of Latium (controlled 100% of holdings in it)
Duchy of Tunis (controlled 100% of holdings in it)
County of Rome (held)

Forming the WRE will also automatically rename the Byzantine Empire to the Eastern Roman Empire.
As the Holy Roman Empire claimed to be the legitimate successor to the WRE, they will not be pleased. Whoever is the Holy Roman Emperor will get a strong claim on the WRE. If the person who creates the WRE also holds the HRE, the HRE title will be destroyed.
2. Relations between East and West
Upon the creation of the WRE, the ERE (if the two Empires aren't held by the same person) will offer tributary status to the WRE. If the WRE accepts, they become a tributary, but with low taxes, and both Empires being capable of calling the other into wars. If it refuses, then the Eastern Emperor gets a strong claim on the West. Both Romes will also have a special casus belli against the other to put this type of tributary status on each other.
Tributary Status:
Tributary pays 5% of income to suzerain.
Tributary can be called into suzerain's wars.
Suzerain can be called into tributary's wars.
Suzerain gains +3 prestige per month.
Tributary relationship breaks if either party no longer holds either the WRE or ERE title.
3. Forming the Roman Empire
To form the Roman Empire, as you can guess, you must own both Romes. The requirements would be:
Titles needed:
Western Roman Empire (held and at least 70% controlled)
Eastern Roman Empire (held and at least 70% controlled)
Duchy of Latium (100% controlled)
Duchy of Thrace (100% controlled)
Duchy of Sicily (100% controlled)
Duchy of Tunis (100% controlled)
Duchy of Antioch (100% controlled)
Duchy of Athens (100% controlled)
Duchy of Alexandria (100% controlled)
Doing this would destroy both the WRE and ERE, and form the Roman Empire, getting you the Augustus trait and all that other good stuff. Both Empires would also lose all de jure territory, with the new combined Roman Empire holding the de jure land of both Empires. The Roman Empire should also give twice the prestige of a normal Empire. (To make up for losing both the WRE and ERE)
There is another important part of doing this. Obviously, the Holy Roman Empire will not be pleased at this. Doing this will give the HRE a choice. Give up it's Empire status, or fight to stop it. If the HRE chooses to give up, the HRE will be destroyed, and a titular "Kingdom of the Romans" will be created in it's place. This obviously means all King rank vassals of the HRE will become independent, significantly weakening it.
If it chooses to fight, they keep the HRE, at least temporarly. When this happens, the Emperors of Rome and of Holy Rome will become rivals, and will get casus bellis on each other. Both Emperors will get a special casus belli on each other, called:
End Roman Pretender:
If Rome wins:
All territory held by the HRE that was historically part of the Roman Empire will be usurped or vassalized by Rome.
The Holy Roman Empire will be destroyed, replaced by the Kingdom of the Romans.
If Holy Roman Empire wins:
Empire of Rome is destroyed, replaced by the Byzantine Empire.
Holy Roman Emperor usurps or vassalizes all territory in de jure Kingdoms of Italy, Sicily, Sardinia and Corsica, Venice, Genoa, Pisa, Naples.
If White Peace:
Both sides lose significant amounts of prestige.
4. Special Roman mechanics
As well as the mechanics currently held, like Born in the Purple or the Imperial Reconquest casus belli, Rome should have some more mechanics.
(a) Roman tributary: Rome should have a casus belli to make tributaries. They will give 10% of income, but more importantly, will give a large amount of levies. These levies will be the same time as the tributary's cultural retinues. This is to represent auxillaries.
(b) Imperial Subjugation: A casus belli that can be used by the Roman Emperor once per lifetime. It costs 5000 prestige to use this casus belli. It will forcefully vassalize an entire Kingdom rank title that was once part of the Roman Empire. The catch is that, naturally, all Kings of territory once part of Rome will, naturally, be threatened by this. They are all allowed to, and likely will, join the war on the Kingdom's size. So, its likely that when Rome uses this, they will end up at war with all of the former empire. Losing also costs a ridiculous amount of money and prestige.
(c) Pratorian Guard honorary title: An honorary title for the leader of the Pratorian Guard. This title will give the holder a seat on the council. The higher their martial/intrigue is, the better chance they have of helping to stop assassination attempts against you. Of course, they, like the spymaster, also have a better chance of plotting against you.
I know this stuff isn't likely, not least because they would be changing an old DLC, but I'd love it. Maybe in a potential Hellenistic DLC?
