Coming from the developers of CKII, too often am I disappointed by the prince not having any kids or wife. This is often the case when the current ruler marries young and has a child before they're 25 or even 20. The heir will age and age, and never have family descendants. It simply happens too often. Being that CKII is this game's historical predecessor, it's really silly.

I try using add_heir and add_consort, but they have 0s in all monarch skills. Is there extra syntax I miss? add_heir 6 6 6, for example, but that doesn't give skills.
 
well, that's impossible. Are you running any mods? I would do a reinstall.
It happens a lot. The chance is repeatable multiple times in one game. Sometimes a sibling becomes heir, sometimes they're really young suggesting a relation to the former heir, and sometimes the heir has a wife, sometimes wife and kid. Too often I find a really old heir with no heirs preceding him. This happens so often when the current ruler marries young and has an heir around 20. Either it happens often, or it becomes annoyingly apparent, being that there was so much time to build up the family tree.

How can I give add_heir and add_consort monarch skills, and maybe personality trait too?

Are you saying that it happens a lot or it happens always - because that's how I understanded your first post.
if you want good ruler type event 9469.
I guess it always happens when the heir and ruler are not so far apart in age and both are past their 40s.
