For some reason I have no smoke on the map for either disease or sieges

Because the smoke is supposed to appear on provinces with disease or sieges and its not for me. Sounds like a bug

Perhaps, not necessarily though. Could be a DLC conflict issue for example.

You should revisit the sticky threads about making reports. There is very limited information for QA to track your issue, you should also provide the save game and a screenshot of the province in question for quick reproduction.
