Here is a save I made before this occurred. What happened is I invited the guy who eventually became emperor to my court to be my steward. No idea if this fact is relevant.
On another note, I am also a secret catholic, even though I have secret cults off. Is this working as intended? Also, when I embrace Italian culture, the rest of my family does not convert with me.

Thank you for this save, I have had a look and cannot seem to reproduce this issue from the save.
Do you have steps from this earlier save to reproduce the issue?
To address your other issue, while you have the game rule turned off for secret cults, it is still possible to have a secret religion you just cannot form a secret cult from it.
As to the Italian Culture issue I see you have made another forum post for it so I will address that there.

I cannot say exactly how it happened. I told you what I noticed at the time, though I cannot be sure these facts are relevant because I cannot experiment using iron man. Here is what I noticed: The man who eventually became emperor I invited to my court to be my steward. I'm not sure how he became emperor, whether it was by faction or he just inherited. Also, he was raising taxes at the time prior to becoming emperor. All of the claims seem to be within de jure Italy. Those outside are those I fabricated earlier.
