Mods: sketchy cheat menu, HIP interface and coat of arms mod, and culturally different cities.
Long story short, the game was going normal, but I found that when I checked some of my vassals, they had conquered land that was not de jure theirs in another enemy country. My empire laws are total vassal war forbidden. I also found that having max council power is no match for the desire to have the oldest inherit, as even after choosing elective for each kingdom title, they somehow are able to switch it back. (I though it was from having allow_laws apply to my vassals, but I found that was not the case, it still happening even if I turn it off.)
I can't really reproduce it, as it seems to happen almost randomly in regard to my vassals starting wars against the law.
