This is not strictly a bug so much as a balance oversight.

When provinces are seeded in later start dates there are no hospitals placed. This is an issue because hospitals slow the spread of disease and reduce de-population. In the early start dates, CM and Old Gods, the lack of hospitals is much less of a problem but as provinces develop in later starts disease spreads more quickly and larger armies appear to cause greater devastation.

The difference is particularly marked when starting just before the Black Death hits as the disease will run rampant in months and utterly devastate the whole map, then persist even after the devastation due to the lack of hospitals to treat it. At the same time without advanced hospitals doctors are less likely to get good events meaning that you not only have a lack of medical infrastructure, you have a lack of good medics. You really notice the difference after the Black Death as few provinces prosper and in some starts virtually none boom without pre-existing medical infrastructure.

Then there's the expense of fully upgrading a hospital, which takes years, and huge amounts of gold.

As I understand it provinces are seeded based on province tech - if that's so then the fix would be to seed hospitals at a certain level of Construction Tech and then assign them some upgrades as that tech progresses.
