If I mark many characters as "special interest" using the star, everything in the interface becomes noticeably sluggish. Even panning around the map is reduced to several FPS. I am playing on Windows 10.

I incorrectly interpreted this as "late game lag" or "large realm lag" for several years. But recently I figured out that it was connected to the size of my special-interest list. Changing the outliner settings to not list characters of special-interest makes the lag disappear, but collapsing the outliner does not.

I have attached my save, in which 454 characters are marked as special interest.
