[2.6.1.1][SKBW] Titular Duchy of d_dyn_reb_518901 in Lombardy(/Italy?)

Description: Playing a Lombardy count run, was fighting some nasty heretics in Ferrara when this Titular Duchy showed up at some point. Not sure if it's related to the war I just won or not. No description for it, but the screen seems to pan towards Ancona when I open it. So that might be where it's linked to?

DLC's: All of them.
Mods: None.

Reproduction Steps: No idea, seems to have appeared at random. Haven't tried reproducing due to lack of personal play time.

It's not harmful I suppose, but it doesn't look nice and probably shouldn't be appearing if it doesn't have a name assigned. See attached save file if you wanna mess around with it.

Same kind of bug here. Playing as the king of Pomerania, a spurious duchy was available for creation (see screenshot)

I did not try to actually create the title

Playing on 2.7.1 [BPAH], this problem still exists - see screenshots.

This is all related to the AI, it's not an area I've been involved with at all. Let me know if you'd like a separate bug report / save file uploaded.

No problem. So, playing on 2.7.1 [BPAH]:
Description - as above, titular duchy name is broken. I've only noticed with the county of Dyrrachion, but for reference also attach screenshot of the Duchy of Dyrrachion, which appears unaffected.
DLCs - Charlemagne, Legacy of Rome, Sons of Abraham, Old Gods, Republic, Sword of Islam, Monks and Mystics. Various CoA packs, dynasty shields, portrait packs and music packs.
Mods - none
Reproduction steps - been on-going for a number of game years, and have saved the game, closed, re-entered.

I have seen this many times.
Somehow rebel title is not destroyed properly after the rebellion and is available to creation.
If you exit and enter the game again all would be normal again.
You can't see it by loading any save.

Thanks but I've tried exiting and reloading and makes no difference for me. It's been there for a few weeks (real life), and will be able to provide screenshots from different dates when I get back onto my personal laptop.

Hmm... I always quit and reload the game when it happened the title usually stop showing.
But I played Ironman.
