Playing as the Emperor of Byzantium I began handing out counties after a recent holy war. The first few vassals became counts like they were supposed to, but after that new characters became lord mayors or metropolitans (they were only granted castle holdings, and were unlanded prior to the grant) with random frequency but outnumbering proper counts. My character at the time was over his vassal limit, and I suspect it may have been tied to that as after I created some new titles to lighten the load new grants SEEM to have gone back to working properly.

I have all of the DLCs except Orchestral House Lords, play with Sunset Invasion disabled, and use no mods.

I can't reproduce the problem reliably as it seemed to happen at random.
