Prisoners catching diseases get healed on release. If your capital province gets a disease, your prisoners can also be affected. On release of such prisoners, if their home province doesn't have the disease, they get healed immediately when arriving there on release.
I would suggest that instead of getting healed, they would spread the disease to their home province. In that way, you can use releasing infected prisoners as a sort of biological warfare (he-he).
