Titular titles currently do not have their de jure lieges saved in the savegame (i.e. they don't stay when you save, close the game, and reload), though those with de jure lieges defined in the landed titles or the history (even after the start date from where you started your game from) will have them saved properly. I believe a similar issue exists and has been reported with dynamically created titles.
For demonstration, here's a simple mod that includes a single titular duchy (d_test) with an event that makes it de jure under France after one day. It works fine initially, but once you save, close the game, and reload, d_test now has no de jure liege despite having been de jure under France before.
Edit: To be clear, if it has a defined de jure liege in the history during the playable time (so for vanilla until 1453) (or if it has one in the landed_titles, obviously).
