Here's the steps to reproduce:
1) Have a King-level Republic (in my case, Venice)
2) Create a new Empire using the decision, which correctly becomes your primary title
3) Die, winning the election for the Republic
4) The Republic becomes your primary title, and you can't make the Empire your primary again because it's not the same tier, meaning forming the empire was essentially useless.
