Here's an annoying bug that has has occured in multiplayer several times (not sure if reproduceable in a non-ironman singleplayer campaign - I haven't tried it). If a player marks someone as a character of special interest and then logs out for long enough for this character to die, he/she stays in the watchlist of the player's character (and his heirs). Normally, a character of special interest is automatically removed from the list upon death, but, for some reason, it doesn't happen while the owner of the list is controlled by the AI. The only way to remove a character from the list manually (that I'm aware of) is through the right-click menu. Since this menu is unavailable for dead characters, they end up sitting in the watchlist for eternity.

The host of the game (not me) had all the DLCs and no mods running. Not everyone in the multiplayer session had all the DLCs purchased, but I did.

EDIT: Confirming that it is reproduceable in singleplayer: Start a game > Add a bunch of old people to special interest > Save game > Switch to another character > Fast forward a couple of decades > Save again > Return to the original character > Everyone who has died will be stuck in the watchlist.
