Black flags usually happens when the game fails to generate them properly. Go to the CK2 folder in "Documents\Paradox Interactive\Crusader Kings II" and delete the gfx folder. This will force the game to generate them again and should then appear correctly in-game.

Deleting the gfx folder should solve the issue if the flags of different titles (like the kingdom of Sweden). If it's the different dynasty coat of arms that are black, that would be a separate issue we are aware of. It should be fixed in the upcoming patch, though. Hope that helps!
