This pertains to the new title creation decisions that were added in Charlemagne (Which are awesome) - I'm calling them dynamic titles for reasons that will be clear in a moment, dunno if that's the official name.
In most cases, when you take the "Found Kingdom" decision, the title you get is a titular title with no land associated with it. Of course, titular titles can gain de jure land by holding it for 100 years, like all other titles. Also, if no one holds the same-level title of some or all of the land you own when you create the kingdom, that land is immediately reassigned to your new title and becomes de jure land in your new kingdom.
The bugs show up when you save and quit the game. It seems all dynamic titles lose their border color information on saving and reloading - the border colors from the kingdom heraldry are replaced by the realm map color and white. If one of these dynamic titles is conquered completely and destroyed, it gets even worse - in the "De Jure Kingdoms" view, the realm color is now completely white, and the name of the kingdom is gone and replaced with k_dyn_{numbers}. The title behaves just like a normal title and you can recreate it if you meet the normal requirements, but the color and name information are gone.

Steps to Reproduce:
1. Start a new Charlemagne game as High Chief Senulat of Slavonia
2. Give yourself the requirements to create a kingdom:
give_title d_pecs 168292
give_title d_croatia 168292
prestige 1000
cash 300
3. Go to the intrigue screen and take the "Found Kingdom" decision.
4. Go to the De Jure Kingdoms view. Note that, since the Kingdom of Croatia does not exist, the Duchies of Croatia and Slavonia are now De Jure part of the new Kingdom of Slavonia. Note also that the Kingdom of Slavonia is a lighter shade of whatever-color-that-is, while the border around it is red, white, and blue, as you'd expect from the heraldry.
5. Save and Resign.
6. Load your save. This time, select Khan Dengizikh of Avaria. Note that the border around Slavonia has changed to that weird pinkish color and white and no longer matches the heraldry.
7. Conquer the Kingdom of Slavonia in a way that destroys the title:
give_title c_varadzin 194006 (note the typo in the title name here)
give_title c_zagreb 194006
give_title c_krizevci 194006
give_title c_usora 194006
give_title c_zachlumia 194006
give_title c_senj 194006
give_title c_veglia 194006
8. Click on one of the counties, go to its realm screen, check the de jure box, and go up to the Kingdom of Slavonia. Verify it has no holder and looks normal.
9. Save and Resign.
10. Load your game. Go to the De Jure Kingdoms view. The place where the Kingdom of Slavonia was is now a white space, and its name has changed to k-dyn-numbers. You can go to it in the realm screen's De Jure view, and oddly enough the heraldry seems to be just fine, but the name is gone.
