I took a look at your savefile and its WAD.
During the election time, there is a value called "respect" which you can find in the republic tab.
If you hover over your mouse on the "respect" value, you will see two values that increases respect.

One is the age factor, the other one is the prestige.

Currently in your save Kalinga has 4863 respect versus your heirs 3610.
If you character dies, Kalinga will inherit the grandcity and all other cities inside the "De Jure Kingdom"

I hope this cleared out for you how the basics of the Merchant Republics system works.

I've read the wiki many times and there's not enough info there. Which De Jure Kingdom? My liege's? If so then why do I lose Korkai as well which is in a different De Jure Kingdom?

The inheritance is working as intended, since the doge will inherit the Republic and the City in that province (Dakhina Desa).
Your heir will still get the provinces outside of the Republic (Cities Rohana, Korkai and the Barony Tirunelveli)
You can still get back the Republic and the City in the next election when the doge dies (If you have the highest amount of respect among all of the Patricians)

OK, that's what SHOULD happen but not what actually happens in this save game. Get the char id and kill yourself and you will see that ALL demesne titles are transferred to the new Doge.

It's been working like this from the start. I owned Rohana very early and lost it. Maybe something to do with Indian succession laws...

I'm going to guess that the issue is related to the Buddhist ability to designate a primary heir. I recall being able to do this early in my game but not able to now.
