In my 2.2.1.1 game I'm currently playing as the kid duke of Mercia who is hated by pretty much all the counts. The strongest (and ambitious) count already hated his predecessor for all of his reign. But I never had a single faction in the realm in about 50 years of play. This can't be normal, can it? The AI seems to get factions and civil wars as normal, an independence revolt totally broke up Charlemagne's realm after his death.
EDIT: I actually got a faction later in the game - maybe I was just lucky.

Unless you've gotten something similar multiple times over the same period? It's bound to happen sometime.
