So here's the situation, i started up a game and selected a random character and immediately entered observer mode to let it run to the end and export the game to EU IV.
I put it in windowed mode so it would run while i did other stuff and after about 100 years i checked what was going on in the world.
I noticed a bunch of counties where under siege and I decided to check what wars where happening.
I then noticed that even though they had taken the holdings they weren't at war, so i used the play xxxxxxx command to switch the king of Castile and noticed that the entire world is hostile to me.
I swapped to a couple of other lords and the same exact thing was happening.

Edit: Ok i reloaded that save and let it play for a little bit and everything seems to be working again and all conquered holdings are back under the original holders control again so I'm confused what was going on.

As Wezqu said, we are aware of the issue, but don't really know what causes it. So if you happen to come across it again and figures out why, any additional information with steps to reproduce the issue would be most welcome.

If you have such a situation reloading will clear it. You can also tell if you are in an 'everybody is hostile' situation because your diploview will have a lot of icons for 'at war due to confliciting wars' (but no tooltips) although the enemy armies are also a subtle clue...

One trigger is winning a war against a faction revolter in another nation while there is another faction revolt going on (so if the neighbouring country has faction wars led by A and B, then winning a war against A will cause you to be hostile to everybody on peace)
