[2.1.4] No River Crossing Between Upper and Lower Silesia

There's a river running through Upper Silesia, and you would have to cross it to get to Lower Silesia, or vice versa. However it does not show up as a potential river crossing penalty when the county, nor is it given when attacking between those two provinces.
I'm pretty sure there's river crossings in neighboring counties along the same river, meaning I'm not so sure this is WAD.

Update: Friend of mine tells me there is also this issue between Pisa and Lucca, with a screenshot of the problem. Apparantly there is a river crossing between Pisa and Piombino, but the river shows up on the map between Pisa and Lucca.

I'm pretty sure there're similar problems up around Ferrara and Bologna.

While we are at it, there is also something odd with terrain penalty betweend India mainland and Sri-lanka. The county in India shows the river icon but no hover text, and the county in Sri-lanka shows nothing at all. (Latest stable patch, without RoI)
