After saving, quitting, and reloading my ironman game, my incapable ruler's regent was removed, and so was the red "no" symbol from his portrait, although he still has the trait incapable.

This is a known issue, which should be fixed in the open beta patch available through Steam. You can opt into that if you want, or just wait for the patch to go live.
