Playing as the Sunni Caliphate starting in 1066 I declared war against my liege, temporarily becoming a king rather than a duke. During this war some of my vassal counts (my vassals from before the war, not other lords that joined the war) created duchies that made me lose my vassals due to them having equal rank to me after the war ended. I lost about half my territory this way. It seems like an easy fix would be to disable title creation if your liege is the leader of a revolt/holds a temporary title.
