I was playing as the Republic of Venizia as the Participazio family and I was a Patrician. I had taken my retinue to a nearby territory in which I owned a trade post and was attempting to take a city. Back home in Venizia, a peasant revolt started and the Doge's army was defeated and before I could get back, the war was over. Here is what the event log said:

The Peasant Revolt for Venezia has ended: Serene Doge Marco of Venice lost!
Giacinto of Venezian Peasant Revolt has usurped the title Grand City of Venezia from Serene Doge Marco the Cruel.
A new great House has risen to the forefront of Venetian politics, under Patrician Leopoldo of di Cremona.
After this, I was given a new character to play, a different member of the Participazio family but in a different Republic (Genoa). Since it is a different republic, the upgrades to my Family House are gone and I control a different set of trade posts. My score was also reset to 0.

I continued playing as Genoa and a little while later ran into another issue. For some reason the game told me I had a new heir, despite my current heir not dying. It said my new heir was a member of another family in the Republic (Marco de Busca) and despite my family member (Agnello Participazio) holding a big lead in respect, it said the expected successor was the guy from the De Busca family. I have attached a screenshot below.

However, this seems to just be a display bug because when my character died, I played as Agnello and he was elected as Doge. The game still lists Marco de Busca as my heir and expected successor of the Republic, even if my family member has a big lead in respect.

I just wanted to add another issue I noticed when playing today.

I was Doge of Genoa. The bug where it indicated that a member of another family would be my heir (as can be seen in the screenshot in the original post) was active. I knew this was just a display bug from previous experience (as explained in the original post) and that I would play as my designated heir as indicated in the Republic panel. As the Doge got older, The Patrician that I had given the "designated heir" title to proved to be a bad choice and I decided to choose a different one. I had the option to imprison the former Patrician of my family (from a previous event) and I did so and then banished him. This freed up the "designated heir" title and I gave it to another family member. However, when going to the Republic panel, the previous guy was still listed as the Patrician of my family. A short while later, my character died and I went on to play as the character that I had designated as my heir.

So to summarize, my heir (the character the game told me I would play as next) was incorrectly labeled (I already discussed this in the original post) but in addition, the leader of my family (who should have been my actual heir) was also incorrectly labeled. Needless to say, these bugs makes it quite difficult to manage your succession and elections, I wasn't able to view the respect for this character and add to his campaign fund, so the Doge position went to another family.

Another update on my problems with Republics. My character died and my heir lost the election, so I was a Patrician under the new Doge from another family. I started a plot to fabricate a claim on the Grand City of Genoa and successfully pulled it off. I then declared war on the Doge, won the war, and forced my demands. It gave me control of Genoa, but it completely destroyed the Merchant Republic of Genoa instead of making me the new Doge. All my trade posts are gone, my mansion and all its upgrades are gone, and the Republic of Genoa doesn't even exist. I am now Count of Genoa and the game is pretty much over. This doesn't seem like the correct behavior. :(

I've had this problem too, but it seems to update properly if you save, resign and reload the save.
Thanks for the info! :) Unfortunately, I was playing Ironman so that wasn't really an option, but I will keep it in mind.
